How do I know whether my trigger ran on a submission?

Triggers are a feature of Enterprise plans.

To troubleshoot triggers or to see what triggers ran on your submission, go to a form submission within your Submissions area and click "Advanced".

Then choose "Events". 

This will show you the events surrounding the Submission, and any trigger "failures" that may have occurred. Generally speaking triggers will fail to run on a submission if they do not meet the criteria specified in the submission, or if there is a field that is required that doesn't exist in the submission.
