Tips for building checklist forms

  1. Use a segmented control field as this means you can require a positive or negative answer.

    In most cases you probably want the options to be "yes" and "no".

  2. Use the text block for long questions.

    Use short labels for the field names/labels. This ensures the user can see the full "block" of text, and ensures your CSV export has short label identifiers.

  3. Use the instructions field to provide instructions to the end user.

    The final printout won't show the text from this field.
  4. Make use of your sections to make it easier for end users.

    Split your checklist into groups of say, 10-20 questions so that it's easier for users to navigate through the list and see where missing data might need to be completed.
