Tips for building checklist forms

- Use a segmented control field as this means you can require a positive or negative answer.
In most cases you probably want the options to be "yes" and "no".
- Use the text block for long questions.
Use short labels for the field names/labels. This ensures the user can see the full "block" of text, and ensures your CSV export has short label identifiers.
- Use the instructions field to provide instructions to the end user.
The final printout won't show the text from this field.
- Make use of your sections to make it easier for end users.
Split your checklist into groups of say, 10-20 questions so that it's easier for users to navigate through the list and see where missing data might need to be completed.
